To provide all levels of home care service, allowing
the client to be in their home and involved in establishing, implementing
and evaluating services.
To provide a coordinated team approach to therapeutic
and rehabilitative health care services utilizing nursing, home health aides,
medical social services, physical, occupational and speech therapy, nutritional
consultation and a system of referral, follow-up and evaluation.
To provide home health care services and training sufficient
to ensure the client and their caregiver the assumption of personal responsibility
for health and personal needs.
To provide educational opportunities within the health
care system for employees through an ongoing participation in professional
organizations.
To provide coordinated liaison with health and welfare
agencies to meet the needs of the community through joint efforts, meetings
and community awareness programs.
